Documentation required by Bandwidth	Letter of Authorization (LOA) (both the Voxbone (is now Bandwidth) LOA and Intelligent Solutions LOA are required). The last invoice of the numbers that are requested to be ported. The last two bills issued by the provider that list all numbers to port – this invoice will usually show the PAC code for each individual number. Copy of the photo ID of the end user (residential port) or legal representative (business port). If it’s a business: A copy of the "Certificate of Incorporation" A copy of the "Certificate of Directors and Secretary of the Company"	The PAC codes are needed for all porting.
