Please complete the Account Change Questionnaire if you’d like to request an account name change for any of the following reasons:
Simple name change – the company is changing its legal name but tax ID and state of incorporation remain the same
Merger – the company is merging with another company
Buy-out – the company is being purchased by another company
Transfer of service – the company is moving/has moved out of the office space and a new tenant would like to take over the service
Other – please explain
Once you’ve submitted the requested information, we’ll generate the customer paperwork to complete the change and will send it to you for signature. We’ll also include execution instructions for the paperwork. The change will be effective once the countersigned transfer documents are returned from Bandwidth.
